  Notice on Fee for Services

Charges based on $115.00 per hour

Retainer Fee: $300.00 per year

Initial consult consists of review of all the information sent to me, and compiling additional questions to ask during our interview, either in person or by phone. If in-house, additional assessments may be performed to obtain all the necessary information to formulate your comprehensive written report of recommendations for your physician. This includes all additional calls each week until all medication changes, doses and symptoms are resolved and completed. This process normally involves about three (3) hours of time and the fee is $350.00.

Follow-up visits are based on an hourly charge of $115.00 per hour

Retainer fee: Includes unlimited telephone calls to me at any time of day or week anywhere I am. You are billed for this service each year if you choose to participate. It is important that I am informed each time you go to the doctor and are prescribed additional medications or treatments that may disrupt your current drug therapy management plan. Also, any problems that arise during the year are shared with me for immediate response. This has been a very popular service to our client base for many years.
